The $495 includes: • Certification account creation. • Application administration. • Commodity code assistance. • Inventory of office equipment. • Document maintenance, quality assurance, & support. • Resume. • Bank Resolutions. • Letter of explanation for capital contribution. • Personal net worth statement. • Communication with the county. Documents needed from you • Bank signature cards and, for corporations, bank resolutions a. Bank signature card must have the number of individuals with access to the bank account and the authority they have in the account. • Birth Certificate. • Copies of three (3) contracts, purchase orders or invoices showing work performed by the business. • Current licenses (Professional, city, county, state, federal). • Signed copies of current leases and/or recent tax bills for properties utilized by the business. • If applicable, proof of bonding capacity and certificates of insurance. • Titles or lease agreements for any vehicles issued to the business. • EIDL Loan documents. • Illinois Secretary of State Letter of Good Standing (current). • Title(s) of automotive equipment for all vehicles used for business purposes or leased through the business (if applicable). Documents (that the county requires) that are included with your $495 fee paid to Howard Tax Prep LLC . • Resume • Bank Resolutions. • Letter of explanation for capital contribution. • Current Personal Net Worth Statement(s) for qualifying owners who represent 51% or more of ownership in the business. Documents we can assist with for an additional fee through our sister company www.howardtaxprep.com/pricing • 5 years of tax returns. • Five (5) most recent year-end balance sheets and income statements for the business (if business has existed fewer than five years, then include year-end balance sheets and income statements for all years of existence.
